Social Worker PACT

While functioning as an independent professional, the incumbent is under the direct supervision of the ACS Social Work Supervisor and receives clinical, administrative guidance/mentoring as it relates to Social Work practice. The social worker is expected to make independent judgments and decisions in providing services to veterans, regardless of complexity and difficulty of the problem presented. He/she is expected to seek consultation when needed. Duties include, but are not limited to:
Establish and maintain effective therapeutic relationships with highly complex Veterans in the Primary Care Clinic (Lahontan Valley Outpatient Clinic) and/or their families. Work independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial problems Complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families Assess functional acuity of the patient and provide appropriate interventions Facilitate action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner. Implement treatment modalities and evidence based practices as well as provide educational classes, and/or supportive groups for Veterans and families, including shared medical appointments Use and teach Veterans in the effective use of the My HealtheVet (MHV) system. Must possess skill in using computer systems, especially the internet for Veterans who choose this modality to communicate with the team. Provide consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and Advance Directives Provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning and compliance with treatment Maintain knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies Provide case management services to Veterans and their families throughout the continuum of care. The incumbent will be able to appropriately utilize principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness Communicate effectively, both orally and in writing with people from varied backgrounds Work Schedule:

United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. English Language Proficiency. Social workers must be proficient in spoken and written English inaccordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. In addition to the basic requirements for employment, the following criteria must be met to qualify for this series at the GS-12 grade level Senior Social Worker, GS-12 (1) Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. (2) Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Ability to work independently with Veterans and their families who are experiencing a wide range of psychosocial problems. (b) Provide social work in the Patient Aligned Care Team context often occurs over a series of several visits. (c) Knowledge of the PACT team approach and the role of each health and allied health care team members as well as an understanding of the role of the social worker in the PACT model. (d) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (e) Complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. (f) Assess functional acuity of the patient and provide appropriate interventions. Utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable. (g) Participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions. (h) knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance abuse disorders, common medications and their effects/side effects, and medical terminology References:

The work requires extreme patience and controlling of emotions. The work may also require assisting physically disabled veterans. Day-to-day work involves sitting, walking, standing and carrying light objects, such as medical records. Work is typically performed in a clinical setting that exposes the incumbent to the communicable disease common of the homeless population. Much of the work is performed in a setting away from the office, where the environment cannot be controlled.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
